Over the last 10 months, ECC have been capturing and reviewing customer journey data after discharge from an acute setting into a short-term intermediate care type service and then into long term service provision. We are now ready to take this to the next stage. The url below provides further insight into the opportunity and the technical aspects of the requirements. The Council would appreciate it if interested parties would complete and return the questionnaire. This will enable us to refine the requirements further.
