Successful applicants will be required to work collaboratively with the CCG to provide high quality care to patients registered with a Northamptonshire GP who have been assessed as meeting the eligibility criteria for NHS continuing healthcare, NHS Funded Nursing Care or who are on a D2A pathway. The aim of the procurement will be to put in place a structure to support the purchasing of placements and to provide quality care to enable patients to meet personal outcomes as described within the service details within a tiered pricing arrangement.
