NHS Midlands and Lancashire Commissioning Support Unit (CSU) is working on behalf of NHS Southport & Formby CCG and NHS South Sefton CCG who wish to award a 6-month contract for an Independent provider of ophthalmology Services for patients living in the CCG areas. The aim of the service is to deliver: * Diagnostic and screening procedures * Surgical procedures * Treatment of disease, disorder or injury Sefton's health care is commissioned by NHS Southport and Formby CCG and NHS South Sefton CCG with Sefton MBC providing social care for the whole area. This is a joint contract award by both NHS Southport & Formby CCG and NHS South Sefton CCG.
