## Notice

Surrey County Council is looking to procure a number of Collaborative Reablement Providers for their new Collaborative Reablement Service. Surrey County Council's Reablement Service can be described as a short term service delivered within the clients own home for up to 6 weeks focusing on re learning or developing new skills, regaining confidence and utilising personal strengths to maximise independence. The Service responds to referrals from both the hospitals and also from the community teams.The procurement will be conducted under the Light Touch Regime inaccordance with Regs. 74-76 PCRs and will consist of 5 lots.

### Lot Information

Lot 1 - East Surrey

This is for the delivery of Collaborative Reablement into the East of Surrey.

Renewal: Contract will include an extension of up to 2 years (1+1)

Lot 2 - South West Surrey

This is for the delivery of Collaborative Reablement into South West Surrey.

Renewal: There will be an extension of up to 2 years (1+1)

Lot 3 - Mid Surrey

This is for the delivery of Collaborative Reablement into Mid Surrey.

Renewal: There will be the option for an extension of 2 years (1+1)

Lot 4 - North West Surrey

This is for the delivery of Collaborative Reablement into North West Surrey

Renewal: There will be an option to extend for a further 2 years (1+1)

Lot 5 - Surrey Heath

This is for the delivery of Collaborative Reablement into Surrey Heath.

Renewal: There will be an option to extend for a further 2 years (1+1)
