## Notice

NHS Devon CCG, on behalf of NHS England and NHS Improvement South West (NHSE/I SW) seeks Expressions of Interest (EOIs) from providers who have the capability and capacity (to be assessed as part of initial procurement process) to deliver Primary Care Medical Services from existing facilities in Plymouth (the Mayflower Group). The service will provide the full range of primary medical care services through an APMS contract with an expectation to provide a variety of Locally Enhanced Services delivered through an NHS Standard Contract. EOIs are also welcome from existing GMS or PMS contract holders. The contract term is yet to be determined but will be for at least 9 years. The annual contract value is approx. PS3.76m based on 2021/22 values. The contract is due to commence April 2022. This funding is based on core GMS funds. It may be possible to consider additional, transitional funding that may be available following agreement of a robust business case that outlines the transitional steps that will be undertaken. Such funding would be based on open book accounting to reflect actual costs incurred and would only be available for a limited period of the contract. Interested parties must be eligible to hold an APMS (Alternative Provider Medical Services) contract and appropriate CQC registration. The Key requirements, that you must confirm you are able to meet are: * Recent evidence of working at scale providing integrated health services within a community setting * On line and face to face services MUST be provided * Recurrent delivery within core GMS funding * Demonstrable knowledge of the patient population to be served and demonstrable evidence of delivering health care services to a similar group * Proven track record of recruitment and retention of clinical and support staff, with robust workforce plan * Recent evidence of high quality service delivery, which might include positive regulatory assessment * Demonstrable ability to provide high quality primary medical services * Evidence of managing significant service improvement and delivering change in a challenging environment EOIs received will be used to help NHS Devon CCG finalise their commissioning and contracting approach for these services. This process is being managed by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioner(s).

Lot 1

This PIN as a call for competition is initially published as a notice for expressions of interest. The Commissioner may then choose to proceed directly to a process with parties who have responded to the notice and there shall be no further notice published. Should NHS Devon CCG proceed to a process it is anticipated to consist of two stages: (1) Stage 1 The aim of this shortlisting stage is predominantly to test the capability and capacity of potential Providers and to identify whether there is more than one capable provider; (2) Stage 2 will either be: (a) a competitive process (if more than one capable provider is identified at Stage 1), which may involve dialogue and will involve assessment of bids against published evaluation criteria and weightings; or (b) a collaborative "Assurance Process" with a single provider, if only one capable provider is identified at Stage 1. Both approaches will involve evidence being considered by subject matter experts and assessed against critical success factors and expected minimum standards. The services are healthcare services falling within Schedule 3 to the Public Contracts Regulations 2015 ("the Regulations") which are not subject to the full regime of the Regulations, but are instead governed by the "Light Touch Regime" contained within Chapter 3, Section 7 of the Regulations (Regulations 74 to 77) This process is being managed by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioner(s). Additional information: Summary Specification It is expected the successful provider will meet all aspects of the medical services contract. Local requirements include: There is a need to provide General Practice (GP) facilities to the population on the current Mayflower patient list. GP provision will need to be sustainable and long-term. The provider, will need to work with NHS Devon Clinical Commissioning Group (CCG) and community services to help shape primary medical services in line with the NHS Long-Term Plan and NHS Devon's Primary Care Strategy. Primary Care Networks (PCN) build on the core of current primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care. Clinicians describe this as a change from reactively providing appointments to proactively caring for the people and communities they serve. Where emerging primary care networks are in place in parts of the country, there are clear benefits for patients and clinicians. Locally defined outcomes The appointed provider will contribute to achievement of high-level outcomes as set out in the Devon Integrated Care System (ICS) Strategic Outcomes Framework within the following categories: * More people will be living independently in resilient communities * More people will be choosing to live healthy lifestyles and less people will be becoming unwell * People who do have health conditions have the knowledge, skills and confidence to better manage them * The healthcare system will be equipped to intervene early, and rapidly, to avert deterioration and escalation of health problems * More care will be available in the community and less people will need to visit, or be admitted to, hospital * People will have far greater control over health services and will be equal partners in decisions about their care * People who need treatment will be treated effectively and quickly in the most appropriate care setting * People will go into hospital when necessary and will be discharged efficiently and safely with the right support in their community.
