The overall aim of the service is to help people with dementia and their carers across the county of Suffolk (West and Ipswich and East), to live independently, safely, and well, for as long as possible within their communities. This includes helping people to manage the impact dementia has on their lives, make informed choices in planning for their future, and put in place practical solutions to stabilize situations and avoid crises. The service will have a complete understanding of the Suffolk health and care system-wide offer of services and systems of support, as well as the demographics across the county. High quality, consistent advice and information will be offered to anyone accessing the service, appropriate to their needs at the time of contact, and paced according to their requirements. The services will be subject to a block contract for 2 years with the potential to extend the contract for up to 1 year; more details are available within the procurement documents.
