This will cover South West Hampshire primary care networks only. A Home Visiting Support Service for Primary Care was initially commissioned in April 2020 in response to the COVID-19 Pandemic to support primary care resilience with General Practitioner home visits across the geography of South West Hampshire System - covering all 9 Primary Care Networks. A review of the service has highlighted the value of the support the service has given to primary care. It is now regarded as essential to the resilience of the sector, especially given the challenges of ensuring equity of access to primary care, both for the ageing population and, patients needing but unable to travel to, a face to face consultation.
