## Summary

The NHS Sheffield Clinical Commissioning Group (CCG) seeks to improve health outcomes for individuals with learning disabilities through the tender titled "NHS Sheffield CCG - Physical Health Outcomes - LD." This open procurement process, classed under health services, aims to provide targeted outreach to enhance annual health checks and vaccination access for this vulnerable population. The procurement is currently active, with the deadline for submission set for 17 December 2021, covering the Sheffield region (UKE32). The awarded contract is expected to foster significant engagement with local healthcare practices to improve service delivery.

This tender presents an excellent opportunity for businesses specialising in health services, particularly those focused on social work and supporting individuals with disabilities. Companies capable of delivering community outreach and education on health matters, vaccination preparation, and health planning would be well-positioned to compete. Additionally, SMEs involved in public health initiatives are encouraged to participate, as there is potential for long-term partnership development through possible contract extensions for enhanced service provision.

## Notice

People living with learning disabilities (LD) face some of the greatest health inequality gaps in England. On average men with LD die 23 years earlier than men without a LD and for women it's 27 years earlier. Poorer physical health is for the majority of people due to preventable illnesses and in part due to physical health needs being overlooked. The Service will provide targeted outreach to support and empower people with learning disabilities to get their learning disability annual health check, to understand their health action plan, and to access vaccinations, in particular flu, pneumonia and covid vaccinations. The commissioned service will: Increase the number of people who been supported to receive their LD annual health check. Increase the number of people supported to understand their personalised health action plan Increase the number of people who been supported to receive their covid vaccination Increase the number of people who been supported to receive their flu vaccination Increase the number of people who been supported to receive their Pneumonia vaccination Increase the number of GP practices engaged in the project and reporting positively about the impact of the project. Be perceived as a positive experience by people with learning disabilities.
