Lincolnshire aims to support and meet the needs of children with disabilities within the family home and in their local community environment via:- 1) Specialist activities not requiring a social worker assessment: a) Early Support Learning Provision for 0-5yrs delivered out of Lincolnshire's Children Centres/Family Hubs b) Targeted Positive Activities for 5-18yrs during weekends and school holidays in the community across Lincolnshire. 2) Specialist provision following a social worker assessment and identified need: a) Domiciliary Care and/or social inclusion for 0-18yrs.
