## Notice

The service will enable and support people to live as independently as possible in their own homes. The care and support will have a person-centred approach that enables individuals to 'live a good life' by using assistive technology wherever possible to promote independence. This contract is for 2 element of the Help to Live at Home: 1. Home Independence and Crisis Support, which includes hospital to home, intermediate care, restorative support, hospital prevent, emergency response 24/7 and crisis support. 2.Home Independence Service, which includes long term support, palliative/end of life support and night support.

Lot 1

Due to the urgency to replace this contract and ensure service continuity, the council undertook a Open Procedure restricted to 2 suppliers with the capability and capacity to deliver the contract. The award of this short term 15 month contract will allow the Council to carry out a PCR15 compliant procurement for a longer term contract.

### Procurement Information

Extreme poor performance of the existing supplier necessitated the termination of the contract for zone 2 and the need to urgently replace the supplier to ensure service continuity to vulnerable people living in the community. This short term contract will allow the council the time to run a full PCR15 compliant procurement process for a new longer term contract effective Sept 23.
