Suffolk County Council is undertaking Market Engagement prior to conducting a procurement exercise for the award of a contract to provide a supported discharge service for Adult and Community service and NHS.This service is not for domiciliary home care as no regulated activities will be required as part of the service delivery. The service will be run out of both Ipswich Hospital and West Suffolk Hospital.The aim of the Supported Discharge from Hospital Service is to support people (adults 18+) discharged from hospital (acute and community) or A&E or an interim care home bed which supports hospital discharge. The Service will help people transition from one of these beds to their homes, by helping them plan for discharge, arranging for them to be transported home if necessary and providing time-limited practical support so they can maximise their independence, self-manage their health and wellbeing and remain connected with their communities.The Service will provide practical support to people in a person-centred way, tailored to reflect people's individual needs, taking account of their gender, age, ethnicity, religious beliefs, disability, and sexual orientation. The service will acknowledge and respect the importance of these areas of people's lives. The supported discharge service will offer support or advice for people to help get them back into their normal routine by helping them to regain confidence and independence. The Service will provide comprehensive training and support to volunteers and paid/qualified employees to ensure they work effectively in partnership to deliver the Service.The supported discharge service will offer support or advice for people to help get them back into their normal routine by helping them to regain confidence and independence.
