## Notice

We are urgently seeking expressions of interest for providers, or a consortium of providers, who are able to provide additional assessment and rehabilitation beds for patients in Leeds. These beds will meet the health and care needs of patients over the age of 18 who are currently in hospital but no longer require acute hospital care, but who cannot be immediately discharged to their own homes without a further period of bedded recovery, rehabilitation and assessment. Nursing, personal care and therapy input will be provided in a flexible, multi-disciplinary way which adapts to meet a range of requirements depending on the assessed needs of each patient. Some patients will require more intensive rehabilitation and some will require a period of recuperation and recovery with a re-abling approach. The aim of the service is to maximize the potential for each patient and to work with partners to facilitate the timely transfer of patients back home or into a long-term placement at the earliest appropriate opportunity. We will expect the provider to actively plan with the Multidisciplinary team to ensure goals are clearly set and length of stay is minimised. Social Work support will be provided by the Leeds City Council Community Bed team.

Type of service required We are seeking additional assessment and rehabilitation beds for patients in Leeds. These beds will meet the health and care needs of patients over the age of 18 who are currently in hospital but no longer require acute hospital care, but who cannot be immediately discharged to their own homes without a further period of bedded recovery, rehabilitation and assessment. Types of patient Our patients vary in their needs and dependency, and you would be expected to be able to support a number of patients at any one time who require 2 or more to transfer. A number of these people will have dementia related needs alongside their physical recovery needs, and we would expect the provider to be able to manage their needs holistically. Support and training will be available via City partners if required. Options for inclusion Therapy: Ideally the provider will have appropriate OT and physiotherapy staff embedded within their multi-disciplinary teams. If this is not possible, system partners would work with you to identify options for an appropriate in-reach model, drawing on staff from across the system to ensure consistency. Therapists would be expected to work alongside other provider therapists in consistency of practice, goal setting etc. Primary care cover: ideally providers would have a relationship established with a primary care partner who would be willing to support them with the temporary registration, 'clerking', ongoing primary care needs of their patients until they are discharged to their permanent practice. Our preferred model would be to commission a single service model with a clear integrated set of governance arrangements. If this is not possible, we would support providers in trying to identify a GP practice or Primary care network who might be willing to support. Size of service: We are looking for units of no fewer than 20 beds, recognising that we need to build relationships between providers and those placing patients, and that multiple smaller units are difficult to coordinate. Our experience is that it is harder to deliver this cultural change in small groups of beds within a long-term care setting, as the purpose of these beds is different. Venues: We are looking for providers who have accommodation, ideally with single rooms and with appropriate shared space for rehabilitation/ active engagement. If you have the ability to provide a staff team, but not a venue, please also let us know. Start date: We are looking for at least some beds to come on-line from November/December 22 in support of the expected difficult situation we will face this winter. The Services to which this Procurement relates fall within Schedule 3 of the Public Contracts Regulations 2015 (the 'Regulations'). As such, the procurement of the Services is being run as a bespoke process akin to the competitive procedure with negotiation. The WY ICB in Leeds does not intend to hold itself bound by any of the Regulations, save those applicable to Schedule 3 services.
