## Notice

The NHS SEL ICB is seeking expressions of interest for the delivery of peer-led structured education courses for people living with more than one long term condition(s) mental health courses and courses for carers in the London Borough of Southwark. . The Service will be for people registered with a General Practitioner or resident within Southwark. Services will be provided in accessible locations across the borough to ensure equitable access for all Southwark residents. The service will also be available via an online platform. This will be a hybrid model of face to face and online model. Personalised care is one of the five key priorities within the NHS Long Term Plan, which states that 'people will get more control over their own health and more personalised care when they need it.' Supported self-management and peer-led structured education is part of the NHS Long Term Plan's commitment to make personalised care the norm. The SEL Integrated Care System (ICS) Development Plan (last revised in March 2022) sets out the next chapter in the development of an 'integrated care system' for South East London. The ICS is a partnership and has identified the six system-wide priorities for improving care: * Preventing ill-health and supporting wellbeing; * Compassionate, whole person care, delivered in community wherever possible; * Rapid access to high quality specialist services when people need them; * Joined up care across health and other public services; * Addressing health inequalities; * Building resilient communities. Self-management support / peer -led structured education is a key element in the first and second priority and enables the last three priorities. Self-management is considered to be effective in improving health outcomes and is supported by clinical studies. Supporting and educating people to look after themselves improves thier motivation, the extent to which they eat well and exercise, management of their symptoms, and clinical outcomes; and is evidenced to reduce their use of NHS resources. Following the programme, participants will have a better understanding of how they can support themselves as active self-managers. Lot 1

The primary aim of the service is to support participants to become active self-managers and to help them engage with their health care professional to be a partner in their patient journey. The service is also available to carers of patients who are unable to access the course themselves so that the carers can facilitate effective self-management of themselves and for the person that they are caring for to prevent further deterioration. As part of the provision courses for carers will be piloted. Secondary aims include: * Enhance patient choice * Maximise completion rates of Service Users * Maximise uptake across Southwark residents * Reduce health inequalities across Southwark * Improved ability to manage their day-to-day life with coping strategies and breathing exercises. * Improvement in mental health and wellbeing * Increased physical activity of service user * Reduction in weight of service user The contract will be offered for a period of two and a half years (2 1/2 years). The contract length will be a maximum of 2 1/2 years. The total Estimated value of the contract (over 2 1/2 years) is between PS190,000-PS229,000 Interested organisation must express an interest via Pro-contract e-tendering portal. It is a requirement that the model of delivery is a hybrid model of both f2f and online courses. The supplier must work within the mobilisation timescales as set out in the EOI documentation. This means only these suppliers can bid for the opportunity. Please note that the contract(s) will be awarded without further publication. Interested organisations must express their interest as instructed in this PIN notice if they wish to take part in this procurement process. The services to which this advert relates fall within Schedule 3 of the Public Contracts Regulations 2015 (the Regulations) and the "Light Touch Regime" (LTR) governing the procurement of Health, Social, Education and Other service contracts. Neither the reference to the "Open Procedure" or the use of the term "EOI", nor any other indication shall be taken to mean that The Authority intends to hold itself bound to any of the Regulations, save those applicable to Schedule 3 services under the LTR.
