They must enable Individuals to gain, maintain or regain skills to help them live independently and/or recover from illness. The aim of the Service is to prevent unnecessary admissions to hospital and residential care. The Provider(s) are to support individuals who are to be discharged from hospital or move out of long or short term, building based care.
