Adults between the ages of 40 and 74, who have not already been diagnosed with one of these conditions, can be invited (once every 5 years) to have a check to assess their cardiovascular risk and be offered support and advice to help them reduce or manage that risk. The contract will run from 1st August 2023 for 3 years until 31st July 2026 with the option to extend for two additional years of 12 months each.
