Pathway 2 - intensive, bed-based rehabilitation (usually 21 days), requiring a community hospital or similar nursing home setting, possibly leading to long term care support package most probably at home. Pathway 3 - intensive, bed-based rehabilitation (usually 28 days), requiring a community hospital or similar nursing home setting, expected to lead to long term care package most probably in a nursing or residential care home. The vision of the model is to maximise independence, de-escalate care as soon as possible to prevent de-compensation and over-time this is expected to increase the number of people managing independently or with care support at home and reduce the numbers of people relying on bed-based care in the short term and long term.
