### Lot Information

LOT 1 - Vision Loss Holistic Services

The Vision Loss Holistic Service provides support and advice to adults who experience sight loss in Liverpool. This includes the provision of Visual Rehabilitation. Visual Rehabilitation is provided to adults who are registered as Severely Sight Impaired or Sight Impaired or will be eligible to be so in the future

Renewal: 3 year initial contract, with extensions of another 1 year, then another 1 year - maximum 5 years (3+1+1)

LOT 2 - Hearing Loss Holistic Services

The service will undertake holistic assessments of needs with individuals who experience hearing loss, the assessment and provision of equipment, provision of advice and guidance as well as the delivery of low-level support.

Renewal: 3 year initial contract, with extensions of another 1 year, then another 1 year - maximum 5 years (3+1+1)
