The successful provider(s) will be expected to work in partnership with health and social care services and the voluntary and community sector to promote personal independence, improved health and wellbeing and to prevent homelessness and reduce factors leading to repeated homelessness. The services are divided into two areas referred to as Lots. The potential provider(s) can bid for one or both Lots. Lot 1 - High Wycombe, High Support. The Service will support 60 service users aged 18 - 55 accessing homeless hostel beds and dispersed accommodation. Lot 2 - Young People services - Medium Support. The Service will support 15 service users aged 16-25 accessing homeless hostel beds and dispersed accommodation.
