This social prescribing service allows older people to be both healthy and independent whilst reducing the burden on NHS services in Sunderland through a reduction in need for GP visits, Out of Hours Contacts, Emergency admissions etc. Essence The Essence Service (the Service) provided by Age UK Sunderland (the Provider) will bring together patients with a formal, medical diagnosis of dementia in the early and early onset stages to create a supportive environment for them to retain their confidence, personal interests and wellbeing. The service is titled 'Essence' as it aims to keep the Essence of the individual focussing on the patient, rather than their dementia diagnosis and look to nurture and maintain their sense of individuality whilst responding to their developing dementia needs. The needs of carer will be considered both in relation to their caring responsibility and their own personal needs. Social Focus The project offers a chance for people aged 50+ with mild to moderate depression, anxiety or stress to get together & participate in various activities.
