Lot 1

The Provider will work in partnership with other members of the Multi-Disciplinary Team to provide: * Specialist care for persons with long term clinical conditions; * Specialist care for older people; * Multidisciplinary/multi-agency assessment and care management planning; * Return to home and follow-up care arrangements; * Active, goal-based rehabilitation, enabling people as far as possible to return to their previous level of function and independence; * Intermediate/integrated care; * Post-operative rehabilitation and recovery; * Medical assessment and treatment; * Continued specialist therapy; * Investigations to aid diagnosis; and * Palliative and terminal Care.
