London Borough of Barking and Dagenham (LBBD) is looking for a single provider with expertise in delivering a therapy based reablement service that is focused on supporting residents to recover from a crisis in their lives. Supporting residents to: * Regain the skills to self-care and continue to live in their community outside of the care system. * Or enable the resident with the minimum level of support to continue to live at home and retain a level of independence.

Renewal: The contract shall be for an initial term of four (4) years with the option to extend by two further one (1) year periods to a maximum contract length of Six (6) years.
