### Lot Information

Lot 1

To increase staffing provision by 3 RTC to provide additional care resource in people's own home - enabling care packages to be extended for up to 6 weeks for some more complex End of Life Care (EOLC) situations, this will enable timely discharge from hospital, supporting EOLC wishes even in complex situations and preventing avoidable hospital admission / readmissions where more specialised EOLC home care is required. Increase by an additional care co-ordinator to support administration of care packages, to proactively support discharge teams e.g. A& E discharge; hospital discharge teams / transfer of care hubs / D2A etc to ensure effective discharge home for Wandsworth EOLC patients. An Award has been made under Direct Award A form 1/4/2024 to 31/3/2025. The lifetime value of the contract is PS90,000.
