### Lot Information

Lot 1

to provide a key community-based resource that supports the avoidance of unplanned hospital admissions for patients with palliative/end of life care needs. This service seeks to put robust arrangements in place for End of Life medications to be easily available for patients being cared for in their usual place of residence. The service will provide access to an agreed stock list of palliative care medicines from one or more community pharmacies, 24 hours/day, 365 days per year. Participating community pharmacy / pharmacies will deliver prescribed palliative care medication to the patients place of residence within 2 hours of prescription being received by them. To ensure a standardised service across Wandsworth, participating community pharmacy/ pharmacies will be required to continually stock an approved stock list of palliative care drugs (that may include specified brands of drug).
