The service will develop, maintain, and deliver a range of person- centered and coordinated services for individuals in Hillingdon. The Service Provider will be responsible for ensuring safe, high quality, non-discriminatory services appropriate to the diverse needs of the Borough. The service needs to work in collaboration with the Council and the wider Health, Care and Community sector to utilise the assets available in Hillingdon to support individuals to live well within their communities. The value of this notice is based on the contract term which is 5 year with an optional 3 year of PS230K per annum.

Options: There is an option to extend the contract by up to 3 years after the initial 5 year period.
