Regular MDTs will be held to discuss patients' needs and identify support required.

NHS Humber and North Yorkshire ICB is intending to award a contract an existing provider following direct award process C of The Health Care Services (Provider Selection Regime) Regulations 2023. Approximate lifetime value of the contract is PS846,998 Contract term - 1 April 2025 to 31 March 2026 (contract term may be extended by 1 year)

Options: Contract contains an option to extend by 1 year.
