The focus is to maximise the Person's independence by providing support with daily living such as personal care and practical tasks/activities. People are encouraged to develop their confidence and skills to carry out these activities and tasks for themselves or with a level of support that is appropriate to their individual needs, maximising the use of community services. The Service will be responsive in ways that encourages the young person to achieve their own aspirations rather than to fit into what is available, supporting access to and making use of community services in the area.

### Lot Information

LOT 1 North Durham

LOT 1 requires accommodation to be situated in one of the following towns or villages or their surrounding area: Beamish Ouston and Urpeth Pelton and Grange Villa Chester-le-Street North Chester-le-Street West and Pelton Fell Chester-le-Street South Bournmoor and Great Lumley Sacriston and Waldridge Burnopfield Dipton North and Tantobie Medomsley and Shotley Stanley North and Kip Hill Annfield Plain North and Dipton South Craghead and South Stanley Annfield Plain South and South Moor Leadgate North and Consett East Delves and Leadgate South Consett West and Castleside Lanchester Langley Park Cornsay and Satley Bearpark and Witton Gilbert Esh Winning and Ushaw Moor West

Renewal: 2 periods of 12 moths are available as allowable extensions based on performance against KPI's, funding and Commissioner approval.
