The overall aim of the HIA service is to enable mainly older adults and disabled people throughout Bristol and B&NES to continue to live as independently as possible, for as long as possible, in their own homes. There are four key elements to this service: * Outcome focused aids, housing maintenance and adaptation services to enable people to live independently at home * Hospital Discharge services to enable people to be discharged from hospital * Information, advice and signposting services * Hoarding support services (Bristol only) ### Lot Information Lot 1 Renewal: Extensions are optional and will be let on a 1+1 basis. The estimated total value of this contract (inclusive of any options to extend) is PS5,759,007.00 (initial term = PS3,265,486; optional extensions = PS2,493,521) ### Planning Information A soft market test will be run to review requirements and ensure they're realistic for our budget. For the market test, we will issue an Early Engagement Notice complete with a questionnaire for suppliers alongside an outline of our requirements to provide the suppliers with context as to what we're asking for and why.
