Clinicians onboard have conversations around diabetes/pre-diabetes risk, whether they are registered with a GP (and encouragement to do so), as well as providing onward follow ups with GP for diabetes/hypertension/NHS check, other health interventions and sign posting towards council and VCS supported H&WB initiatives.

The lifetime value of the contract is PS64,800.
