## Notice

The London Borough of Hammersmith and Fulham is seeking to commission a Carers Hub to fulfil its statutory duties under the Care Act 2014. This includes responsibilities related to the provision of information and advice, emergency planning cards, training, support planning, social opportunities and the facilitation of Carers Assessments and reviews for adult unpaid carers aged 18 and over including those transitioning into adulthood. The Council is seeking the provision of a one-stop shop 'Hub' model to enable Carers to access services that support their physical, mental and emotional wellbeing to enable them to continue in their caring role and to improve their quality of life. The Council expects the successful Provider to develop, maintain and deliver a range of person-centred and coordinated services for carers in Hammersmith & Fulham and will be responsible for ensuring safe, high quality, non-discriminatory Carer services appropriate to the diverse needs of the borough.

Carers Hub Contract
The London Borough of Hammersmith and Fulham are seeking suitable providers to deliver services to unpaid carers in the borough.
The Carers Hub contract supports us to meet our Care Act (2014) duties to carers. Services that the Carers Hub currently provides under this contract are:  
• Carers assessments, reviews, support plans andpersonal budgets
• Social events and peer networking/support  
• Information, advice and training on a range of topics.  
• Signposting to other services where appropriate 
• Forging partnerships with local charities and businesses to enhance the offer to carers.   
Referrals for an assessment can be made to our Carers Hub from the Council, other professionals or as a self-referral. Council social workers are also able to conduct carers assessments as part of their work with the cared-for, but our Carers Hub provider does around 80% of the total annual carers assessments.
We are holding a market engagement event 1pm-2pm Thursday 1st May online using Microsoft Teams. We hope you will be able to join us but we will send the slides and a survey to any interested providers who are not able to attend.
