## Notice

This contract provides services for people living with memory impairment within Bath and North East Somerset (B&NES) to enjoy living the life they want to; that they have opportunities to contribute something back, that they ultimately have choice and control over the opportunities they wish to participate in, and that they have access to good quality support that can meet their needs at the right time and in the right place. Services that are reliable, flexible, and focused on the individual are required. Services should help to improve people's quality of life support independence and, if possible, prevent a person's needs from escalating. Provide support to compliment the support of the networks of families, friends, neighbours, and carers who all contribute to those who need care and support. The aim is to enable people living with dementia, who need care and support during the day to live as independently as possible whilst enjoying meaningful activities based on their skills, interests, wishes, and needs and which promote their mental and physical wellbeing. Focus on empowering people and supporting them to stay connected to and feel part of their communities, help to keep people well, and delay or reduce the need for more intensive interventions.
