The GP Out of Hours Service will * work with health and care system partners across primary, community, mental health, acute and social care to provide support and advice to individuals with the aim of avoiding A&E attendances and hospital admissions, where clinically appropriate. * Provide a face-to-face service and telephone-based treatment and advice to patients where NHS 111 triage has determined that a more in-depth clinical assessment is required by a GP/other professional, or home visit is required. * Provide acute management of exacerbations of long-term conditions, including supporting people to use any prescribed rescue medication in line with care plans. * Provide bookable appointments directly by both NHS111.
