Description of the services to be provided: Establishment of an End of Life hub to manage all End of Life referrals, providing a single point of access, increase capacity in 'hospice at home' services to deliver palliative and end of life care within the North Nottinghamshire community supporting patients to die in their preferred place of care. This notice is an intention to award a contract under the most suitable provider process. The approximate lifetime value of the contract is PS4.8 million per annum (PS14,400,000) for 3 years. This is a new service with an existing provider The estimated dates between which the services are intended to be provided are 1 December 2025 to 30 November 2028. Additional information: In line with the Health Care Services (Provider Selection Regime) Regulations 2023
