Lot 1 - Ceredigion

An Arts & Health programme (visual arts and crafts/mixed media programme) will be offered to patients who regularly present at GP, this is a new service, which is due to commence in June 2026. surgeries in Ceredigion, Pembrokeshire and Carmarthenshire living with a chronic condition and/or pain, social isolation and mental health difficulties as a result of circumstances/condition to improve health and wellbeing. The project will be delivered by an experienced arts organisation/s with a track record of delivering high quality arts provision to improve health and wellbeing. The expected outcomes are as follows: Improve wellbeing Enhance health outcomes- reduce stress, improve mental wellbeing, and increase Self-management of chronic conditions Reduce demand on clinical services Increase social inclusion Foster patient activation- providing skills, information and confidence in living well with illness. this notice is an intention to award a contract under the competitive process.

Renewal: Has an option to extend for a further twelve (12) months
