NHS Somerset Integrated Care Board ("the ICB") intents to award a contract to an existing provider, following a Provider Selection Regime Direct Award Process C The existing service will support High Intensity User (HIU) patients referred from agreed locations or through virtual channels, and will provide home visits where appropriate. Taking a holistic approach and working collaboratively with clients to identify root causes of high attendances and address these through coaching techniques that promote future resilience. The contract value is PS75,450. The contract will commence 1st April 2026 for 12 months expiring on the 31st of March 2027 with the option to extend for a further year. This notice has been published by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioners.
