The model is designed to bridge long standing barriers and strengthen opportunities, responding to the gaps identified across Oldham's system: families still need support that is timely, accessible and close to home; commissioned services must deliver greater value; and families want advice from trusted professionals who understand their strengths, culture and lived realities.
