## Notice

The procurement relates to the provision of a Tier 2 Adult Weight Management Service for residents of Sandwell. The service will deliver a structured 12-week behavioural weight management programme aimed at supporting adults with overweight or obesity to achieve sustainable weight loss and improved health outcomes. The programme will include nutritional education, physical activity as a core component to the programme by including access to leisure centres, a minimum of level 3 training /level 4 personal trainer for safe exercise with diabetes and obesity, behaviour change support, and regular monitoring of participant progress, including weight, waist circumference and lifestyle changes. The service will be delivered through leisure centres and will consist of group sessions supported by trained personal trainer coaches, with the option for individual support where appropriate. The programme will be delivered to cohorts of eligible residents referred through agreed referral pathways, including primary care, public health services, and self-referral where appropriate. The provider will be required to deliver the programme in line with national guidance, including NICE guidance on overweight and obesity management, and to collect and report agreed performance and outcome data, including programme completion rates, engagement levels and against three significant outcomes of waist circumference, reduction in body fat percentage or 5% weight loss outcomes.
