Social Prescribing is an established model of care designed to enhance both mental and physical wellbeing. It forms a core component of Universal Personalised Care, empowering individuals to take an active role in managing their health through tailored, community-based interventions. This approach supports a wide range of people, including those: - living with one or more long-term conditions - requiring support with their mental health - experiencing loneliness or social isolation - facing complex social needs that impact their overall wellbeing The London Borough of Camden intends to award a contract for the period of 12 months to the three providers, as set out below: 1. Brandon Centre (North Partnership) 2. Fitzrovia Youth in Action (South Partnership) 3. Catch 22 - the Hive (Youth Lead Social Prescribing Activity development)

### Procurement Information

This is a Provider Selection Regime (PSR) intention to award notice. The awarding of this contract is subject to the Health Care Services (Provider Selection Regime) Regulations 2023. For avoidance of doubt, the provisions of the Public Contract Regulations 2015 and Procurement Act 2023 do not apply to this award.
