The Camden Adult Pathway Partnership (CAPP) is a small clinical team that provides nurse-led outreach to 16 supported accommodation sites across Camden, known collectively as the Adult Pathway. The service provides a health outreach model with a multi-disciplinary approach for single homeless people with complex needs, including ex rough sleepers. Following a direct award process, in accordance with Direct Award process C under the new Provider Selection Regime (PSR), which came into force on the 1st of January 2024, the Council are proposing to award a contract to University College London Hospitals NHS Foundation Trust (UCLH) for a period of 12 months from 16th April 2026 until 15th April 2027 for a total cost of PS310.904.
