## Notice

West Dunbartonshire Health and Social Care Partnership is developing a service which meets the needs of older adults aged 65 and over who are going through diagnosis, those who have received a diagnosis and those who have been living with a diagnosis of Dementia for some time. The Brain Health and Dementia Resource Centre contract will provide post diagnostic support through the provision of Post Diagnostic Support Link Workers who will support individuals for a minimum of 12 months post diagnosis, this is in line with the dementia post diagnostic support local delivery plan standard. The service will support individuals to have the necessary assistance and support to achieve and sustain their individual outcomes.

Lot 1

The primary outcome of this service is to deliver a residential short breaks service for adults with learning disabilities, complex needs and associated conditions, their carers and families and to strive to achieve the outcomes set out in the individuals care and support plan. This service is for adults with a diagnosed learning disability with a varying complexity of support needs, which may include health needs. The Service Specification for this Service sets out the requirements relating to adults with diagnosed learning disability aged 18 years until age 65 who may also have physical or neurological conditions; acquired brain injury; enduring mental ill health or life limiting conditions.
