Age Connect West Midlands; VCFSE Coordinating Lead for the Fairer Futures Dementia Support Service, for 24 months at a total of PS66,979 Alzheimer's Society; Culturally inclusive dementia engagement and support project; for 24 months at a total of PS53,285 - description of the services to be provided;

Options: Dependent on the continuation of funding and achievement of the key performance indicators, the contracting authority is able to award an extension.
