The services will provide support for adults in Hammersmith & Fulham by proactively taking all opportunities to build on people's strengths, maximise their independence and connect them with their community. This procurement represents a strategic redesign of the previous service model. It reflects learning from previous delivery, stakeholder engagement, and evolving adults social care priorities. The new model will have a focus on reablement enabling support to residents to regain the skills, confidence and social networks to return to their previous levels of independence as far as possible and reduce the likelihood of admission/ re-admission to hospital, or the need for long term care and support. Scope of Services: The commissioned services will provide intensive, enabling support to residents to regain the skills, confidence and social networks to return to their previous levels of independence as far as possible and reduce the likelihood of admission/ re-admission to hospital, or the need for long term care and support. While many people will not need support after reablement, some people will require further support to enable them to continue to live safely at home.
