The awarding of this contract is subject to the Health Services (Provider Selection Regime) (Wales) Regulations 2025. Anyone not deemed suitable for the switch must be evidenced in the records, consider using the phrase Edoxaban switch not clinically appropriate (no code) or, Edoxaban contraindicated (SNOMED CT:2949391000000118) or, Adverse reaction to edoxaban (SNOMED CT:1957181000006103). Those deemed suitable for a switch proceed to a consultation with the patient or their representative. Consultations to switch edoxaban to generic apixaban or generic rivaroxaban can be done via telephone, virtually or face-to-face.
