The services will be procured through 6 proposed lots: Lot 1: Core Community Homecare Lot 2: Dementia & Mental Health Lot 3: Rapid Response & Hospital Discharge Lot 4: Complex Care & Treatment of Disease, Disorder or Injury (TDDI) Lot 5: Carers Emergency Response Service (CERS) Lot 6: South-East & Rural Sector - Core Community Homecare Proposed contract: The Commissioners intend to procure these services with a planned start date of 1st April 2027. Contract period will be for 3 years plus 2 additional 3 year increments (3+3+3), with an end date subject to extension(s) of 31 March 2036.
