## Summary

Greater Manchester Combined Authority is procuring health and social work services to reduce health inequalities among people experiencing multiple disadvantage, including homelessness, substance misuse, mental health issues, domestic abuse and contact with the criminal justice system. This opportunity suits health and social care providers able to design and deliver inclusive services for people facing severe health and social inequalities. Lot 1 is relevant to organisations with experience providing primary-care-based support, coordinating with GP practices and other agencies, and addressing complex needs associated with multiple disadvantage across Rochdale, Oldham and Manchester. Lot 2 suits providers with specialist palliative and end-of-life care capability, particularly for adults experiencing homelessness. The authority states that both lots are suitable for small and medium-sized enterprises and voluntary, community and social enterprises. Bidders must be able to demonstrate service quality, present their proposed approach and answer interview questions at the second stage, while also addressing the procurement’s social-value requirements.

In Greater Manchester there are an estimated 120,000 people experiencing Multiple Disadvantage. This is defined as a combination of homelessness, substance misuse, mental health issues, domestic abuse and contact with the criminal justice system. People facing multiple disadvantage are at the sharp end of socio-economic and health inequalities. As part of Greater Manchester's Live Well commitment to tackling health inequality, we want to build on the existing Inclusion health service to reduce this inequality. GMCA are looking for providers to deliver two distinct services which will remove a range of different barriers for people experiencing MD to having their health needs met. Lot 1: Primary Care Led Support- Deliver support in a primary care setting to people facing Multiple Disadvantage, in Rochdale, Oldham and Manchester.
