## Summary

NHS England intends to commission specialised paediatric whole-body MRI surveillance for children with Li-Fraumeni syndrome, constitutional mismatch repair deficiency syndrome and rhabdoid tumour predisposition syndrome. The service aims to detect cancer early through regular surveillance and improve care after an inherited cancer predisposition has been diagnosed. Providers would deliver, interpret and report whole-body MRI and brain MRI, establish referral and results pathways, and coordinate with genetics, clinical and imaging services. Work is divided across six geographical areas: London and the East of England, the Midlands, the North East, Yorkshire, the North West, and the South East. Providers must be based in, and deliver services from, the geographical area or areas for which they seek to provide the service. The procurement is for healthcare services.

This is a planned Provider Selection Regime intended approach at the pre-market planning stage, rather than a competitive tender. NHS England expects to use the Most Suitable Provider Process under the Health Care Services (Provider Selection Regime) Regulations 2023, with pass/fail assessment by the National Commissioning Group. The estimated total value is £525,525 GBP. The process is divided into six regional lots, and providers can express interest in the areas they can serve. The buyer expects completed Expression of Interest Questionnaires and Declarations by 12 noon on 17 September 2026. Provider assessments are anticipated between September and October 2026. The Procurement Act Regulations 2023 do not apply. The intended approach is subject to the Provider Selection Regime, with award decisions made by the National Commissioning Group.

This will suit specialist paediatric imaging providers with established whole-body and brain MRI capability, including the ability to scan children under general anaesthetic where required. Relevant organisations will need radiologists experienced in interpreting and reporting whole-body MRI, cancer predisposition syndromes and multidisciplinary review. Providers should have strong referral, communication and continuity-of-care arrangements with consultant clinical geneticists, Regional Genetics Services, referring clinicians, patient support groups and other imaging organisations. They should be able to share clinical and imaging data lawfully across participating organisations and collect outcome and monitoring data. The service also calls for MRI safety, quality assurance and reporting processes aligned with CQC requirements, national standards and NHS diagnostic imaging guidance, plus facilities suitable for diagnostic imaging and links supporting paediatric-to-adult transition.

## Notice

This is an Intended Approach Notice to award contracts using the Most Suitable Provider (MSP) Process for of the provision of Specialised Paediatric whole-body MRI(WBMRI) Surveillance for Cancer Predisposing Syndromes across the following Lots:

Lot 1: London & East of England
Lot 2: Midlands
Lot 3: North East
Lot 4: Yorkshire
Lot 5: North West
Lot 6: South East

Providers must be located in and deliver services from the defined geographical area(s) that they are interested in providing services to.

Inherited cancer predisposition syndromes are genetic conditions that increase the risk of developing one or more types of cancer. This service is for three rare inherited cancer predisposition syndromes; Li-Fraumeni syndrome (LFS), constitutional mismatch repair deficiency syndrome (CMMRD) and rhabdoid tumour predisposition syndrome (RTPS). Published clinical guidelines for these conditions recommend the use of regular whole-body MRI (WBMRI) with the aim of detecting cancers at an early stage.

The overall aim of WBMRI surveillance for children with cancer predisposing syndromes is to provide WBMRI screening for patients with LFS, CMMRD and RTPS. The provision of expanded capacity within these groups for WBMRI scanning aims to improve clinical care of individuals once an inherited cancer predisposition has been diagnosed.

Providers will:
* Provide, interpret and report WBMRI Scanning for the patient cohort described
*Provide, interpret, and report brain MRI Scanning (the first one with contrast when possible)
*Establish effective pathways with referring and relevant clinical services for referral, undertaking surveillance, timely communication and sharing of imaging results, and onward management where required.
*Have established links and pathways with Regional Genetics Services to support referral, coordination of surveillance, communication of results and continuity of care.
*Demonstrate expertise in the provision, interpretation and reporting of WBMRI, including access to radiologists and multidisciplinary review with experience in the management of patients with cancer predisposition syndromes.
*Facilitate communication with referrers including consultant clinical geneticists
*Have capacity to plan and undertake WBMRI under general anaesthetic if required
*Have information governance processes that permit sharing clinical and imaging data across organisations involved with providing a cancer surveillance service, and collection of clinical outcome and monitoring data;
*Engage positively with associated patient support groups.
*Ensure clear communication with Genetics Services and patients, to support patients to make decisions on where to access screening.
*Work collaboratively with other organisations e.g. within imaging networks, to provide scanning and reporting capacity and expertise to meet the needs of their population including transition between paediatric and adult services.
*Operate appropriate Quality Assessment processes in accordance with CQC requirements and relevant national standards and guidance for MRI safety, quality assurance and reporting.
*Provide facilities will be in line with NHS guidance on Facilities for diagnostic imaging and interventional radiology.
*Support continued educational initiatives for non-genetics health
