## Summary

Thames Valley Integrated Care Board intends to commission specialist palliative and end-of-life care for babies, children and young people with life-limiting conditions across the Thames Valley. The service includes specialist medical support, symptom management, short breaks and supportive stays, hospice-at-home care, 24/7 specialist nursing and medical support, and end-of-life care. It will also provide emotional, practical and bereavement support for parents, siblings and wider family members, together with youth and transition services supporting independence and preparation for adult services.
