It will operate as an integrated intermediate model using a predominantly but not exclusively GP and nurse workforce, They will provide: * A service to the Berkshire West footprint. * Central triage and assessment working with RBFT. * Advice and guidance working with RBFT. * Community management of clinically appropriate lower complexity * Conditions. * Outreach to women in underserved communities and marginalised populations. * Onward escalation to secondary care where clinically required. * Integration with patients' GP record where possible.
