Hampshire County Council commissioned extra care housing services in Petersfield. To support recovery from illness, through supporting timely discharge from hospital and preventing unnecessary acute hospital admissions and re-admissions. To reduce residents' social isolation, improve the maintenance and development of their social support networks, facilitate community access and enable them to feel confident living in their community. To meet the fluctuating and developing needs of residents. To reduce to need for onward moves to residential and nursing care.
