## Summary

Liverpool City Council is developing a Home from Hospital service for Liverpool residents after discharge from hospital. The service will accompany patients home and provide practical and social support, including shopping, topping up utilities and befriending. Its purpose is to enable safe and timely discharge, reduce the risk of readmission, promote independence, help people regain confidence and support reconnection with their communities. This is a community health and support service delivered in Liverpool. It is seeking feedback on service design, innovation, partnership opportunities and market capacity. Providers are invited to submit the engagement questionnaire by 21 September 2026 at 12pm. The future service is expected to run from 2 November 2026 to 1 November 2027. The estimated value is £400,000 excluding VAT, or £480,000 including VAT, for one lot. The Council has indicated that small and medium-sized enterprises and voluntary, community and social enterprises are suitable participants.

This engagement will suit providers that can support people immediately after hospital discharge and deliver reliable assistance in their own homes. Relevant strengths include accompanying patients home, arranging or carrying out shopping and utility top-ups, providing befriending, and helping individuals rebuild confidence, independence and links with their communities. Organisations with experience of reducing avoidable readmissions, working alongside hospitals and community services, safeguarding vulnerable adults, and coordinating practical and voluntary support should be well placed to contribute. The Council is also interested in innovative service models, partnership arrangements and evidence of market capacity. Voluntary and community organisations, social enterprises, established care or support providers, and smaller local organisations are explicitly encouraged to prepare their views and shape the future requirements before a formal procurement is advertised.
