Feedback gathered will support the development of the final specification, procurement strategy, and route to market. Suppliers will be expected to be able to assist customers to make more informed decisions surrounding their population health programme. * Support of the delivery of population health management to the NHS and broader public sector. * Consultancy services are about helping the NHS or other public sector organisations that may use this framework to implement and deliver population support for some or all of the people who live in the areas they either commission or provide health and care services to. * Consultancy could extend across the NHS, wider health, public health, local government, broader policy areas impacting on health (such as transportation policy, housing policy, employment, environmental - other areas of policy relating to population health management and social determinants of health. * Consultancy might also be more targeted, dealing with a more limited area of healthcare, such as risk stratification, social prescribing, medicines usage, etc. Lot 3: Combined Solutions This Lot covers the provision of Population Health Management solutions that deliver bundle services or comprehensive workflows. Population Health Management provided under this Lot should ensure a seamless, integrated approach, delivering all the necessary components required for Approved Organisations.
